Carlton
To make an appointment at the Carlton site you must
meet the
eligibility criteria
of the Clinic. You may
- telephone 9349 7455 and request
an appointment
- call personally at the Clinic to make
an appointment
- write to request an application for
an appointment
- complete the online application form
- download an application form and facsimile or post it into the clinic
(Pensioner Concession and Health Care Card holders only)
Your name will be added to our waiting list if you are eligible
and an appointment will be posted to you.
If you require urgent attention, please contact the Clinic
on 9349 7455 and request an appointment
Remember, a referral or letter of introduction from your eyecare
practitioner is required if you are under continuing or current
eye care. In addition, all patients who have attended an optometrist
in the last 2 years are requested to bring a letter of referral
or introduction from that optometrist if appropriate.
Other Sites
General clinic services are also available at the regional clinic sites at Broadmeadows,
East Preston, Doveton-Hallam, Braybrook and Frankston. Services are also available
to Aboriginal and Torres Strait Islanders at the Victorian
Aboriginal Health Service. If you wish to attend these sites, please contact the site directly
to arrange an appointment.
